Cancellation

In home consultations require 24 hours cancellation/reschedule notice. Failure to notify by your portal messaging will result in a $100 cancellation fee.

Travel distance may affect schedule availability and fees may apply for greater distances. We currently travel to surrounding areas of Springfield and Champaign, Urbana depending on availability.

INTAKE FORMS

Complete upon scheduling visit by email or phone

Please register for your personalized client portal and complete these forms no later than 48 hours before your scheduled in-home or virtual visit. This allows us to review your file and make our visit as beneficial as possible.
